| A flight of two RF-101Cs was returning from an aborted reconnaissance mission to the Ha Gia POL storage facility when one of the aircraft (call sign Carbine) was shot down two miles north of Yen Bai airfield. Capt Warren had experienced an instrument failure and had handed over the lead to his wingman shortly before his aircraft was hit and caught fire. Capt Warren ejected and was in voice contact with the pilot in another US aircraft for about two hours. Contact was lost about 20 minutes before a SAR force arrived and nothing more was ever heard of Capt Warren. His remains were handed over by the Vietnamese in September 1986 and positively identified the following December. |
